Confidentiality & Privacy Policy
The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.
Exceptions include:
- Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
- If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
- If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
- A court order from a judge. I will make every effort to protect your privacy in this circumstance and will discuss it with you beforehand.
Missed Appointments/Cancellations
If you need to cancel an appointment and/or reschedule it must be done 24 hours in advance to avoid a $75.00 missed appointment fee. After 2 missed appointments, we reserve the right to terminate continued services. Appointments can be cancelled and rescheduled by calling Mindy at 336-403-7288 or by filling out the “Schedule Request” button under the appointments bar on the website.
Inclement Weather
Mindy Snyder follows the W-S Forsyth County School weather policy. If schools are closed generally we will be closed. Please call though to make sure as sometimes we will still come into the office to see anyone who would still like to come in. There will be no charges for appointments missed and rescheduled due to weather.
Fee Structure and Payments
90791: Intake $130.00
90837/90834: Individual Counseling $110.00
Phone calls, written reports, or
Correspondence: $100.00 (per hour pro-rated)
Co-pays and deductible payments are expected at the time of service by cash, check, or PayPal. If you desire to use a credit card, an additional $5.00 fee will be applied to cover the service charge. You will receive a statement if any of any outstanding balance. Balances are expected to be paid within 30 days. Thank you!
Legal Matters and Fee Structure
Please know that I must be fully informed regarding custody situations. If there is custody agreement, please provide us with a copy of it for our records along with any supporting documents. If we are ever legally requested for court testimony, understand we are representing the child who is the client not either of the parents. Sometimes notes will be subpoenaed. Summary statements will be written when notes are requested and all parties involved will receive a copy.
Non- refundable deposit for court appearance: $500.00 (applied to court fees on original date of court only)
Phone consultations with lawyers: $150.00 per hour
Written reports and correspondence: $150.00 per hour
“Stand by” consultation on day of court: $150.00 per hour
In person court appearance: $250.00 per hour including travel time
Expert Letter of Recommendation for Judge $100.00 per hour, minimum 3 hours
